Resource Specialists

Resource Specialists are professionally trained staff that can help you by providing information about services, resources, and programs which can help you to maintain your independence in the community. Whether you are living with a disability, recently had a significant medical event, or are experiencing changes due to aging or the progression of an illness, a Resource Specialist at the ADRC-CW can help you feel less overwhelmed. Here are some of the services provided by a Resource Specialist:

Options Counseling
A Resource Specialist will meet with you to discuss factors to consider when making decisions about housing and care needs now and for your future. While there are a variety of ways to meet and pay for your long term care, it is important to think ahead about how you will fund the care that you will receive when the time comes.

Transitional Services
As children with disabilities near adulthood, Resource Specialists are available to assist them and their families with determining what services they may be eligible for as an adult and assist them with accessing these services.

Eligibility and Access to Publicly Funded Long Term Care
The Aging & Disability Resource Center of Central Wisconsin is the single entry point for accessing publicly funded long term care program such as Family Care or IRIS. These programs offer a variety of services that include medical and non-medical care to help people who have a chronic illness or disability. A Resource Specialist will work with you to determine if you are eligible, counsel you on the programs choices which are available to you, and coordinate your enrollment into a program.
