RESOURCE SPECIALIST

Whether you are living with a disability, recently had a significant medical event, or are experiencing changes due to aging or the progression of an illness it is easy to become overwhelmed. Resource Specialists are professionally trained staff that can help you.

What We Do:

Information and Assistance.
We provide you with information about services, resources, and programs which can help you to maintain your independence in the community.

Options Counseling.
We will meet with you to discuss factors to consider when making decisions about housing and care needs now and for your future. While there are a variety of ways to meet and pay for your long term care, it is important to think ahead about how you will fund the care that you will receive when the time comes.

Transitional Services.
As children with disabilities near adulthood, we are available to assist them and their families with determining what services they may be eligible for as an adult and assist them with accessing these services.

Eligibility and Access to Publicly Funded Long Term Care.
We are the single entry point for accessing publicly funded long term care program such as Family Care or IRIS. These programs offer a variety of services that include medical and non-medical care to help people who have a chronic illness or disability. We will work with you to determine if you are eligible, counsel you on the programs choices which are available to you, and coordinate your enrollment into a program.


Current State of Publicly Funded Long Term Care Programs
At present there is no wait list for publicly funded long term care programs in Marathon & Wood Counties.
Langlade and Lincoln Counties do have a waitlist for publicly funded long term care programs. Please contact a Resource Specialist from ADRC-CW to obtain current information as to the status of the wait list in your area and criteria to be placed on the wait list.
